| Horizon Behavioral Health service portfolios ensure a continuum of care for individuals and families in Central Virginia. Each portfolio offers a comprehensive array of services to address mental health, substance use and intellectual disabilities. Services Available: Case Management Counseling Intellectual Disability Mental Health Outpatient Care Psychiatric Evaluation Substance Use TreatmentHorizon’s Mental Health and Residential Substance Use Services portfolio provides a comprehensive continuum of evidenced-based treatment to ensure success in recovery. Our substance use treatment services focus not only on recovery, but on whole person health. All Consumers Receive:CASE MANAGEMENT: A case manager helps guide consumers through their process of care. INDIVIDUALIZED SERVICE PLAN (ISP): Clinical team members develop an ISP based on consumer’s needs, signs/symptoms of withdrawal, and risks. GROUP THERAPY: Groups are led by specialized clinicians and provide opportunities for in-depth discussions around the complexity of addiction, its underlying causes, and the essential elements to maintaining sobriety. PEER SUPPORT: Peer support is essential to recovery so we offer individual and group counseling sessions facilitated by Peer Recovery Support Specialists who have completed their peer certification.
